You can submit your forms via email or fax, but they must have a wet (ink) signature to be processed. Electronic or digital signatures are not accepted at this time.
As an eligible member receiving benefits through the U.A. Local 71 Health and Welfare Plan, you can submit your medical and dental claims anytime through the Ellement App. Some healthcare providers also offer direct billing, allowing them to submit claims on your behalf—so you can avoid paying out-of-pocket upfront and enjoy a smoother benefits experience.
